About the Role:
Elderly Savior Homecare is seeking compassionate, skilled, and reliable Licensed Practical Nurses (LPNs) to join our growing home care team.
In this role, you will provide hands-on patient care, support care plan execution, and play a critical role in monitoring patient health and progress in a home setting. This position is ideal for an LPN who is detail-oriented, patient-focused, and comfortable working independently.
Key Responsibilities:
- Administer medications and monitor for side effects or adverse reactions.
- Monitor and record vital signs and patient condition.
- Observe and report changes in patient health, behavior, or progress.
- Assist in implementing and following individualized care plans.
- Collaborate with RNs and other healthcare professionals to support patient care.
- Perform wound care, including cleaning and dressing wounds as needed.
- Maintain accurate, timely patient documentation and reports.
- Educate patients and families on care routines and treatment adherence.
- Ensure a safe and clean care environment.
Qualifications:
- Active Licensed Practical Nurse (LPN) license – State of New Jersey.
- Graduate of an accredited Practical Nursing program (ADN or higher preferred).
- NCLEX-PN passed.
- Minimum 1 year of nursing experience (home care preferred).
- Strong understanding of patient care, medication administration, and documentation.
- Ability to work independently with minimal supervision.
- Excellent communication and interpersonal skills.
